我会继续和诊所沟通以及要求他们或我自己报告到卫生部门的,希望降低以后发生这类事情的概率。
如题,求建议。我的诉求:承认是他们的错误,道歉,整改他们的流程。
不喜欢他们写的report,责任推得一干二净,说是我limited English的问题。
今天去诊所打一周前约好的流感疫苗,结果给我打了阿斯利康的新冠疫苗,而且我还告诉护士我约了七月底的辉瑞新冠疫苗了。
觉得他们程序很有问题,打前没给我看或确认、签证任何文件。我到了后和前台说了约了流感疫苗,医生叫我进去也确认了打流感疫苗,让我等护士。过一会护士匆匆忙忙进来了,和前台是同一个人。打完立马护士感到不对,问我:你是要打流感吗?...
我全程没有签任何东西,也没有给我看任何文件或疫苗瓶子(有时候打流感医生会给看确认打的是流感)。这个我和当值医生提出了质疑,他说因为阿斯利康的covid保存问题,一瓶是多个剂量的量,不能反复拿出冰箱。我提出那应该至少拿文件确认一下,以确保不会因为语言沟通等产生错误。他一直和我说要focus在事情发生后的解决、,你每天出门都会take risk,不用太担心血栓这种低概率事情,我表示这些我都了解,也表示理解了。提出要改善流程,避免再发生类似事情,而且我还有别的健康concern他说会和specialist沟通+research再告诉我。结果我事后拿到的case note上写的原因是因为我limited English,只有一句:the patient came for flu vaccine, was administered AstraZeneca COVID-19 vaccine, 这算是承认他们的问题吗?
关于英语:我电话预约,前台、医生确认,包括和当值医生沟通都是英语。而且我和护士说:I booked Pfizer COVID vaccine in the late of July. 说完这句她几乎马上就给我打了,打完立马反应过来问我:你是要打流感吗?

Thank you for your email.
I apologise if I have in any form given any impression to you or anyone that I am hiding the truth.
The fact remains the same that you came for flu vaccine and been given Astra Zeneca Covid-19 vaccine by error, no matter what anyone write in your notes.
I wished this incidence would not have happened, since it has happened, we would like to provide you the full support at the level of your comfort.
The support offered by me or Dr A or any other staff member who were not involved in the incidence, is not an attempt to deny the fact of the incidence nor shifting the responsibility. It is purely focussed on your wellbeing.
Although your treating doctor was Dr B, and the nurse was C but everyone in this practice including myself are very willing to provide any additional support needed by you.
As you might recall that I found out about the incidence while you & the nurse were talking in the treatment room when I walked in. You became my priority immediately, therefore I delayed & stopped my own booked patients’ appointments. After talking to you, I rang the Covid-19 vaccination centre and few other information centres for the advice. It was taking longer therefore I requested you to continue your already planned blood tests as requested by your specialist and spoke to Dr B while you were still in the premises having your tests. The information I got from the vaccine centre was not sufficient and could not answer all your questions.
You mentioned that Dr A is your regular doctor, so I organised a follow up appointment with him as well while I was trying to get more information about the vaccine in your case.
Dr A has particularly good knowledge of Covid & its vaccination because of his special interest in it and he reads a lot of current research. Even though Dr A was not involved in the incidence, but he was very willing to help you, like me.
I will answer your specific questions raised in your email.
1) I notified the incident to Dr B your treating doctor and the nurse C. Incidence form was filled, and the practice manager and the other Practice Nurse was informed who helped me in finding more Covid vaccination resources.
There is already active review of the process happening and changes will follow to prevent any such incidence in the future.
2) The incidence & your email as well as this letter is included in your notes as an additional evidence of the incidence.
3) The record of vaccination & its batch number is in your file and an immunisation statement from Australia Immunisation Register will be attached to this email. You could alco access these information through MyGov account.
4) & 5) Dr B your primary treating doctor, I and Dr A are still trying to get reliable answer about the second dose and your **health planning.
I would like to clarify here that I did not form any opinion about your interaction with the doctor and nurse involved in the incidence, nor on your or my English, I simply noted down our conversation without any judgement, solely focussing on your wellbeing.
However, I found you very constructive in your approach & suggestions like showing the label of vaccine to patients. I have already adapted this into my practice.
As I mentioned earlier, we are here to help you, but I will leave it to your comfort level that who would you be comfortable with to be in contact and follow up.

诊所说会跟进支持我,目前还没啥说法,出了个report,我要了一份copy,上面没说任何他们的疏忽,只把责任往我身上推

是啊,是属于年轻女性风险最高人群。我其实觉得概率也不会很高,但是不认可他们现在的处理方式,而且觉得这样的事情不应该再发生,要不然下次可能是危险的事呢

安慰一下!不要太担心,你想想疫苗刚开始按种时,有多少年轻人的医护人员和一线年轻的工作人员都打的阿斯利康,百万分之四才会有血栓,而且即使得了也大概率能治好,宽心点,该干啥就干啥,我认识的很多年轻医生护士都打的是阿斯利康,不会有事的。

不管有没有副作用 必须要report。lz可以直接向有关部门投诉吗 有懂得来说一下

打疫苗的流程挺复杂的,要填表,要确认身份,要给你看疫苗名字和过期时间,难道你的就这么直接一针下去了?

倒不是说打了阿斯利康就有多大风险,但是这诊所这么不负责任好过分!

这个绝对是失误了,不管打哪个疫苗都要填表格,签字的。这些流程都没走吗?而且护士也要和医生确认疫苗吧
