You can buy it yourself or though your super fund. The cost is tax deductible.

If you are a clean skin insurance then no need for medical checkup. Also the amount insured will make a difference. If large amount is insured then they might ask you to do medical checkup. Same for life/TPD insurance.
I used have MLC, now I am with AMP.

Not sure if there is a clear cut off line. My FP friend tells me it's really company dependent.
Normally you don't have to. I insured more than you and no check.
